How does someone who is set up to approve a Purchase Order get a notification through his email?

You turn on the approvals by checking Require Approval on the Approval tab on the Purchase Order Preferences screen like this:
But that just triggers the approval process to happen.
For notifications, you have to use the Automation Notifications screen. There is actually a sample of a Purchase Order approval notification in the SalesDemo database which looks like this:
The key is the Conditions tab which is pictured in the screenshot above.
Have you tried Approval -> OwnerID -> DefContactID -> EMail?
