I would like to add the PO approver to the PO Order form, but I'm not sure what relationships to use.

Try joining to EPApproval on POOrder.NoteID=EPApproval.RefNoteID, but be careful because there could be multiple approval records in which case the Purchase Order will get listed multiple times in your result set.
