Questions Error sending approval notification

Kindly assist;

I have set up approval map for Quick Check; added it on AP Preferences with the standard(out of the box) quick check notification.

When I clear the Hold checkbox the correct approver is assigned and status changes to Pending Approval; however the notification is not emailed. the warning displayed is "Unable to process approval notification. Cannot insert the value NULL into column 'NoteID', table 'AcumaticaDB.dbo.UploadFile'; column does not allow nulls. INSERT fails. The statement has been terminated."

Where could I be getting it wrong?
